During this visit, we’ll take the time to understand your oral health needs — whether you’re concerned about periodontal (gum) disease, experiencing receding gums or bone loss, or exploring options like a tooth extraction and implant. Dr. Jensen will perform a thorough evaluation, explain your treatment options in clear, compassionate terms, and work with you to develop a personalized plan tailored to your goals and long-term oral health.

We will submit a claim to your insurance for the consultation fee of $125. Many plans cover the consultation, but depending on your coverage & deductibles you may have a co-pay. See our insurance page for more information on how dental insurance works.

Already Scheduled?

Prior to your appointment, new patient paperwork will be sent to you via email and text. Completing these forms in advance will help ensure a smoother visit. Please bring a photo id with you as well as a list of any medications you take.

If you are unable to complete them beforehand, you are welcome to fill them out in our office. In that case, we kindly ask that you arrive 15 minutes early to allow time for completion.

To avoid any confusion or taking of unnecessary x-rays, please ask your referring dentist to email any relevant information and recent x-rays to records@jensenperio.com.
