We will submit a claim to your insurance for the consultation fee of $125. Many plans cover the consultation, but depending on your coverage & deductible you may have a co-pay.

To avoid any confusion or taking unnecessary x-rays, please ask your referring dentist to email any relevant information and recent x-rays to records@jensenperio.com.

Prior to your appointment, new patient paperwork will be sent to you via email and text. Completing these forms in advance will help ensure a smoother visit. Please bring a photo id with you as well as a list of any medications you take.
If you are unable to complete them beforehand, you are welcome to fill them out in our office. In that case, we kindly ask that you arrive 15 minutes early to allow time for completion.

We allow 50 minutes for your consultation so you have plenty of time to receive a thorough evaluation, ask all the questions you need and talk through your detailed treatment plan.
